Submitting EVV Data to CareBridge via CareSmartz360

CareSmartz360 integrates with CareBridge to streamline your Electronic Visit Verification (EVV) submissions. Follow the steps below to ensure your agency is successfully set up and submitting data.


Step 1: Register Your Agency with CareBridge

All agencies must complete the CareBridge provider registration form:

Step 2: Confirm Technical Setup

Inform your Onboarding Specialist, or other Caresmartz360 contact (CSM or Support) of your registration. Send them a copy if you are able. Then:

The CareSmartz360 team handles this part for you.

  • Our development team will set up a secure SFTP connection with CareBridge.

  • If your agency started with us before December 18, we will also verify that the CareBridge Manual Data Post report is available and that your EVV configuration page includes the required SFTP details and reason codes.

  • If anything is missing, we’ll take care of it internally—no action needed on your part.


Step 3: Complete Your Admin EVV Settings

Once SFTP is set up, your team will need to:

  1. Go to Admin Settings > EVV Configuration

  2. Enter your agency-specific EVV settings

This includes things like time zone, default reason codes, and your provider ID, if applicable.


Step 4: Set Up CareBridge as the Aggregator in Payer Profiles

Next, for each payer that requires EVV submission through CareBridge:

  1. Navigate to Billing > Payer Profiles

  2. Open each relevant payer

  3. Select CareBridge from the “EVV Aggregator” dropdown


Step 5: Reason Codes Will Now Be Required for Manual Adjustments

After setting CareBridge as your aggregator, you’ll be prompted to enter Reason Codes when manually adjusting schedules—especially for actions like manually clocking in or out.

This is expected behavior and ensures EVV compliance.


Step 6: Submit Visits to CareBridge

To send your EVV data:

  1. Go to Reports > EVV > CareBridge Manual Data Post

  2. Use the filters to select the shifts you'd like to submit

  3. Check the boxes for the desired visits

  4. Click "Post to CareBridge"

A successful post will show a hand icon 🤚 in the confirmation column.


Step 7: Check for Errors and Fix Them

To confirm the batch was processed correctly:

  1. Click into the batch

  2. Use the “Show Detail” button to view any errors

  3. Click the red error icon to review each issue

You can edit individual “cards” for visits that failed. Use the dropdowns to select the correct Reason and Action codes to resolve each issue.


Step 8: Export Visits to Claims in the CareBridge Portal

After successful submission, you’ll still need to export the visits to claims directly from the CareBridge portal to ensure payment is processed.
