Purpose
The Notes & Tasks feature provides a centralized way to document important information, create follow-up actions, and maintain a historical record across multiple records within CareSmartz360. Notes and tasks can be associated with caregivers, clients, applicants, prospects, payers, staff members, roles, and other entities, allowing agencies to track interactions, decisions, observations, recommendations, and required follow-up activities in a single system.
When an Associated Party is selected, the note or task becomes linked to that record and will be available when reviewing the associated profile and related reporting.
Understanding Notes & Tasks
Notes & Tasks are designed to help agencies maintain organized records of important events, observations, communications, and follow-up actions.
A Note documents information that should be retained for future reference.
A Task documents an action that should be completed by an assigned individual or team.
The feature allows a single note or task to be connected to one or more relevant records through the Associated Party field, creating a connected history throughout the system.
When Should You Use Notes & Tasks?
Use Notes & Tasks whenever information should be:
Retained as part of a historical record
Shared internally with agency personnel
Linked to multiple records
Included in operational reporting
Tracked for future follow-up

Before You Begin
Prerequisites
Before creating a note or task:
Determine what information should be documented.
Identify all related parties that should be associated with the note or task.
Decide whether the information is informational (Note) or requires action (Task).
Determine whether internal users should be notified.
Creating a Note
Step 1: Open the Appropriate Profile
Navigate to the profile where the documentation should originate.
For example:
Caregiver Profile
Client Profile
Applicant Profile
Prospect Profile
Payer Profile
Step 2: Create a New Note
Open Notes & Tasks.
Select Add Note.
Enter the required information.
Select the appropriate privacy setting.
Enter detailed comments.
Step 3: Select Associated Parties
Use the Associated Party field to link the documentation to additional records.

Caregiver
Staff User
Prospect
Applicant

Associating additional parties does more than identify recipients. It creates a relationship between the note and those profiles, allowing the information to be referenced from multiple locations within the system and included in related reporting.
Step 4: Save the Note
Select Save to create the note.
The note is now associated with the selected records and available for future reference.
Creating a Task
Tasks function similarly to notes but are intended to track work that must be completed.
To Create a Task
Open Notes & Tasks.
Navigate to "Office Tasks".
Select Add Task.
Enter task details.
Assign the task to the appropriate user.
Select any related associated parties.
Save the task.
Using Associated Parties
The Associated Party feature is one of the most valuable aspects of Notes & Tasks because it allows documentation to follow everyone involved.
For example:
Caregiver Disciplinary Documentation
A supervisor documents a coaching discussion with a caregiver.
The note can be associated with:

The supervising staff member
The office responsible for follow-up
This ensures the documentation can be referenced from all related records.
Client Service Concern
A client raises a concern about a specific caregiver.
The note can be associated with:
Client profile
Caregiver profile
Assigned care coordinator
This creates a complete record of the event across all involved parties.
Hiring and Applicant Documentation
Recruiting staff document an applicant interaction.

Sending SMS and Email Notifications
Notifications are a secondary function of Notes & Tasks and are designed to support internal communication about an existing note or task.
Important
The primary purpose of the feature is documentation and recordkeeping.
SMS and Email options may be used to notify internal users that a note or task exists, but the note itself remains the system of record.
To Send a Notification
Open the note or task.
Select Send as Email or Send as SMS.

Verify that the profile was selected in the Associated Party field before saving the note. Associated parties determine where the note relationship is established.
A Notification Was Not Received
Verify:
A valid internal recipient was selected.
Contact information exists for the recipient.
The notification was successfully sent.
Remember that notification delivery is secondary to the note itself. The documentation remains available within Notes & Tasks regardless of notification status.
